Psychiatric evaluation and medication management are available through telehealth for adults located in Ohio.
What Happens Next?
1. Schedule your initial appointment
Use the secure scheduling link above to select an available appointment time and provide some basic information.
2. Complete your new patient forms
Before your first appointment, you'll receive instructions for completing your intake paperwork and providing insurance information, medical history, current medications, and other information needed for your evaluation.
3. Meet from the comfort of home
Your initial psychiatric evaluation will take place through secure telehealth. We'll talk about what brings you to care, your history and goals, and determine together what treatment options may be appropriate.
Insurance & Payment
Opal Meadow Psychiatric Care currently accepts Anthem Blue Cross Blue Shield, Carelon Behavioral Health, Aetna, and Cigna through our partnership with Headway.
Coming soon: UnitedHealthcare, Optum, Medicare, and CareSource.
Insurance coverage and benefits vary by plan. Patients are encouraged to verify their mental health/behavioral health benefits directly with their insurance company before their appointment.
Private Pay
Initial psychiatric evaluation: $250
Follow-up appointment: $150
HSA and FSA cards are accepted. Superbills are available upon request for patients who wish to seek reimbursement from an out-of-network insurance plan. Reimbursement depends on your individual insurance benefits and is not guaranteed.
A sliding-scale fee may be available based on financial need. Please contact the practice for additional information.

Opal Meadow Psychiatric Care welcomes referrals from primary care providers, therapists, and other healthcare professionals.

Reason for referral
Relevant diagnoses and medical history
Current medication list
Recent progress notes or relevant clinical records
Laboratory results or other testing when applicable
Insurance information, if available

Our office will review the referral and contact the patient regarding next steps.
Please do not send urgent or emergency referrals by fax. Opal Meadow Psychiatric Care is an outpatient telehealth practice and does not provide emergency or crisis services.
